Volunteer Registration

By submitting this application, I affirm that I am at least 18 years of age. I affirm the facts set forth in this application are true and complete. I understand that if I am accepted as a volunteer, any false statements, omissions, or other misrepresentations made by me on this application may result in my immediate dismissal. Further, I understand that as a volunteer, I am offering my services of my own free will without any expectation of compensation, benefits, or insurance of any kind. I voluntarily agree to participate in this program and hereby waive, release, and hold harmless from any liability or claims for damages for personal injury, including death, as well as from claims for property damage which may arise in connection with the above-named activity, against the supervisor, SSM Health and SSM Health Cardinal Glennon Children's Foundation and its employees. I hereby agree to the Volunteer Agreement upon submission of this application.
